How to submit a claim document?

If you plan to file a claim with the UCJF, you must submit a Notice of Intention to Make a Claim (“NOI”) within one hundred eighty (180) days of the date of your accident. To process your claim and determine your eligibility for statutory benefits, you will need to submit additional required documents.

Required Documents for Submission of a Claim:

Ensure your claim submission includes the following:
Incomplete submissions will delay the review and decision process.
  • NOI Form
  • PIP Application
  • Affidavit in Support of UCJF Eligibility
  • Certificate of Medicare Eligibility
  • HIPAA Privacy Authorization
  • Proof of Residency (e.g., a recent utility bill or rental agreement)
  • Copy of Police Report (if applicable)
  • Proof of No Other Insurance Coverage (e.g., a denial letter from the insurance carrier of the striking vehicle)

Pursuant to statute, the UCJF can assess member companies writing automobile insurance in New Jersey. These companies cannot recover their assessments through policy surcharges.

In 2003, the New Jersey Automobile Insurance Competition and Choice Act transferred responsibility for the administration of the UCJF to the New Jersey Property-Liability Insurance Guaranty Association (“Association”).

As of January 1, 2004, the role of the UCJF was expanded to provide private passenger and pedestrian PIP coverage when other coverage is not available. At the same time, EMB payments ceased and the responsibility for the run-off of EMB claims was transferred to the Association.
